The information we collect on or through our Website may include:

  • Information that you provide by filling in forms on our Website, including when you request a quote, refer a client, comment on our blog, request a speaker, or contact us. This information may include your name, company, mailing address, state, telephone number, email address, website, role, referral information (including first and last name, company, address, telephone number, email address, website, and any specifics you believe will be beneficial regarding your referral), questions/comments, and any topics you would like requested speakers to cover.
  • Information that you provide in order for us to provide requested professional employer organization services.
  • Information that you provide in conjunction with pay cards and Christmas clubs. This information may include your bank account information.
  • Information that you provide in connection with applying to be an HR referral partner. This information may include, in addition to any information contained in your application, information collected pursuant to a background or credit check and information related to prospective clients.
  • Information that you provide when register for, log into, or engage with your PrismHR employee account. This information may include your name, social security number, employer identification number, personal and work email address, password, username, information related to your time clocks, time cards, taxes, PTO, schedule, pay, messages, events, position, and hire and review dates, employee number, employee type, employee ID, location, division, department, project, shift, benefit group, work group, birthdate, age, gender, ethnicity, nickname, marital status, date married, information related to whether you use tobacco, whether you are disabled, and whether you are blind, home and cell phone number, resident and mailing address, emergency contact information, veteran status, driver’s license number, expiration date, and license state, vehicle make, model, year, registration number, date added, and date removed, information related to your vehicle insurance policy (including carrier name, policy ID, policy holder name, and policy start and end date), information related to your benefits, any documents you upload, and information related to your 401K. When you log into our Prism HR resources as a manager, we may also collect information related to your employees and support team (including information regarding employee details, PTO register, FMLA events, job candidates, employee benefits, organization charts, position and status type history, leaves of absence, position changes, and rehires), employee terminations, new hires, overtime reports, payrolls, client documents, skills, and analytics, invoices, approvals, reports, and messages.
  • Information that you provide when you log into or use your Pricing Point Account. This information may include information related to your activity and sales details, sales representative details, agency details, prospects (including information related to prospect details, payroll, WC, health, files, and tasks/notes), risk underwriting, and CSAs sent for signature.
  • Information that you provide when you apply for employment with Lyons. This information may include your name, email address, telephone number, address, resume, date available, desired pay, website, blog, or portfolio, LinkedIn profile URL, and the name of the person who referred you to the position.
  • Your search queries on the Website.
  • Records and copies of your correspondence (including email addresses), if you contact us.
